Shalom does not own a school bus or van. Historically, parents have provided transportation for players by driving to the away games. Every player must have parental permission to travel with an approved adult. My son/daughter has my permission to travel with an approved adult to and from away games and in the event of an accident will not hold Shalom liable.

In the event I can not be reached in an emergency, I give permission to the physician or supervising nurse selected by the school advisor, to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for my child as needed.
Photo/Video Release*
I hereby grant permission for my child to be photographed and/or video recorded while participating in athletic programs and activities of Shalom Christian Academy. It is my understanding that photographs are the sole property of Shalom Christian Academy and will be used for advertising or promotional purposes (such as Shalom's social media, website, and print publications). I give permission for my child's name to be included for such purposes. Please note: if a parent chooses not to grant consent for photo/video, a student may be asked to step out of a group photo, cropped out of a photo, or have their face blurred for promotional posting or publication.
